I just ran in to this issue, I installed version 1061 and was never asked the ip address or credentials. I right clicked and ran as administrator and was prompted to enter the credentials and ip.

There is no way I am aware to kill the process, but usually it is hanging because there are active mgcp ports off hook. If you do a sh voice port sum, look for off hook ports and go and make them admin down. I just had this happen to me, I shut down ...
