Does the jpegPhoto ldap attribute on AD has photo stored in it? Is it possible that there is similarity in email address (same email for two person) between the two users with similar phone?
Set the Directory service to "None" and take out the username /password from the service profile. UDS don't need them.

Nothing wrong with your html, only thing i say may be add <body></body> after html and img.src won't work if it cannot find the path to the image.
