It's end of 2018 and your solution still works perfectly! Also I did not have to enable or change any MTP or DTMF settings from default on my SIP Trunk configuration on CUCM.
It is Jan 2018 and your solution still works like a charm!! Before I used your solution I tried doing the checkbox on the MTP under the SIP trunk in CUCM, the MOH worked externally but users were not able to pickup calls after placing them on Hold.

Thank you for the suggestion for manually adding in the dial-peer voice 999000 pots to the appropriate ports. That worked immediately for me and now the FXO port is registered.
