C3750-48PS switch - Mode stuck on SYST and no console access

After powering up a WS-C3750-48PS switch, Normal POST LED flashing of lights does not happen, instead the switch is stuck in SYST Mode on the front panel. My attempt to hold down the mode button upon powering up for 15 sec fails to reboot the switch....

Sometimes when I do a "show gatek calls" to view current Video calls, the information show for a particular call shows the H323ID calling itself, however it shows different E.164 address for SRV and DST endpoint. This does not look right to me. Calls...
