Does IOS version 12.3(8)T4 support analog modem module NM-16AM-V2?
In the link: http://cisco.com/en/US/products/sw/iosswrel/ps5207/products_feature_guide09186a00801f129e.html , it is said that version 12.3(7)T is supported, but it is not mentioned if later versions are supported.
Can anyone help me on this one?

Support for NM-xAM-V2 was introduced in 12.3(7)T, that means all images after that release will support the card (in the specified platforms).

mak,
Well it seems that it isn't. I tried 12.3(10)T and still the module wasn't detected at initialization and it was confirmed when 'show diag' command was executed.
The module was finally detected using version 12.3(7)T, the version recommended in the link above.
This resolved the issue.

Can you please send me the exact image name that works & that does not. I will like to try out the same in my lab or will have to look for a possible bug.
Thanks, Mak

Here is some more research on the NM-AM-V2 not being recognized.
In 12.3T timeframe Cisco adopted a newer IOS image bundling which was called (internally) reformation images. For more information on these image sets please check..
http://www.cisco.com/en/US/products/sw/iosswrel/ps5460/products_ios_series_alternate_home.html
For example, c3745-a3jk9s-mz.123-7.T, it isnt supported in this image set. The only platform on which NM-16AM-V2 is supported in this image set is
c3660, as they don't support these new reformation images.
We need to use one of the reformation images, like c3725-ipbase-mz type of images for this.
The following images tested in lab worked:
c3725-ipvoice-mz.123-10.1.T
c3725-entbase-mz.123-8.T3
c3725-adventerprisek9-mz.123-4.XD3

Mak,
The unit's platform is 2621XM.
Here are the images that I've tried:
c2600-js2-mz.123-8.T4
c2600-telco-mz.123-10a

Shouldn't there be a "T" in 123.10a ?
Maybe something like 123.10.T ?

Cisco IOS (tm) C2600 Software (C2600-TELCO-M), Version 12.3(10a), RELEASE SOFTWARE (fc2)
That's the whole version name.

This is IOS c2600-telco-mz.123-10a.bin which is NOT a "T" IOS release.
The link in your conversation start, speaks about "T" IOS release.
This is the reason why using c2600-telco-mz.123-10a.bin you cannot see the module.
You could try a latest IOS "T" release like c2600-ipvoice-mz.123-11.T2.bin

Yes, you're right marinos, I believe my vendor has sent me a wrong version. But what about the other version - c2600-js2-mz.123-8.T4?
Thanks for your suggestion, I'll take note of that.

If you take a look at this link, you'll find some bugs and the affected images, including c2600-js2-mz.123-8.T4
Solution Release Number is 12.3(8)T5, so you could try c2600-js2-mz.123-8.T5.bin

I don't know what platform you are using, but there is another thread going on at this moment that discusses a software forced reload issue with 12.3(8)T and the NM-16AM-V2 on 2600 series.
