Lost UDP packets during calls

Hi,
Can you help me to get rid of this output from command
"sh call history voice brief "

What we need to know is where the packet loss starts (From AA.AA.AA.AA or from BB.BB.BB.BB?) and the meaning of LostPacketRate and lost:X/0/0 values

Would be very helpful to understand who is the root of the problem.
