Is there any reason why 4.1.181 was pulled from CCO? I just upgraded to 4.1.181 on all my 4400s. I'm not experiencing problems on 4.1.181 but would rather know the reason why it is no longer available.

It was 4.1.185.0, just saw 4.1.190.4 in the rel notes page

Some new features & fixes from 181 to 185.
woohoo! time to upgrade again.. I was wondering what to do with all my spare time...

Looks like support for the new 1522 mesh AP is the main feature add for 4.1.190.4
There a handful of closed caveats, as well some of the usual open ones

I'm not positive, but it looks like 4.1.190.4 is a branch of the tree designed just for mesh APs, rather than just a new general release. The release notes have been removed from the site (or at least, I can't find them anymore). Either way, I'm just now getting myself settled in at 4.1.185.0 and they're already prepping a new release? That's the kind of development I like to see. Fast and furious fixes and features! =]
