802.11r Fast Roaming timing out in FlexConnect Mode - works fine in Local Mode?

Have an odd problem we can't seem to figure out. 

 

In FlexConnect mode our clients (apple iPhones) can't seem to roam using 802.11r.  The fast transition keeps timing out, and trying over and over to connect.  This results in the phones losing wifi connection for up to five minutes before rejoining the network via a full auth.  It also results in the clients sticking to very low RSS APs since the fast roaming is not working.  

 

Everything is setup correctly (all APs in same flexconnect group, etc.)

 

In local mode fast roaming works fine and there are not issues, but it presents a separate set of problems for us since we designed the system to be in local mode.  

 

I'm attaching a log of a client trying to fast roam in our flexconnect setup.  Can anyone decipher why this is happening?

 Try to use Over the DS Fast Transition. You can enable it under Layer 2 Secuvirty tab on the WLAN. As per logs, you are using Over the Air. 

 Of course, both should work properly, but, go figure.

Oddly enough, DS fast transition is enabled yet it is still showing over the air in the logs.
