         - FYI : https://bst.cloudapps.cisco.com/bugsearch/bug/CSCve45714
           This one looks problematic for your environment because the fixed releases mentioned can not
   run on this controller model (it is too old)
                    Consider modernizing the wireless environment

 what is the WLC software  version and the AP software  version?

Also need more info like what the others mentioned.  You happen to have other ap's joined to that controller?  Is this something new you purchased for a lab?
