Cisco ISE 2.2 Upgrade with Bootable USB

I'm currently upgrading my 3595 ISE server from USB using http://www.cisco.com/c/en/us/td/docs/security/ise/2-2/install_guide/b_ise_InstallationGuide22.html ; guide.  I downloaded the recommended Live USB creator, but unfortunately, I keep receiving the error, syslinux/vmlinuz is image not found.   I also tried to upgrade from the GUI using the repository using 3cdaemon, but during the actual upgrade process, the preparation upgrade never completes. Can anyone please assist me with a possible solution?

I also had a problem when upgading ISE from 2.1 to 2.2 using the gui.

upgrade was loaded on the system, but stuck in state: something like "no host to initiate upgrade".

finally i reverted to using the CLI and the repository

first I did upgrade cleanup on both systems

then again the upgrade using upgrade prepare and upgrade proceed (nodes one-by-one)

NB! the process (single node) took about 90 minutes..
