Does the antenna ship with a Mast Mounting Kit or does that have to be ordered separately? If a separate kit is required, what's the part number?

It comes with one.... take a look at the one that it comes with and see if that will work for you.

If you need something different, Sparco Technologies can custom make a mount for you. They also have different type of mounts.
