Failure reason: 'The AP draws 15.4 watts from Ethernet'."

How about action on the log page, change the settings as follows: radios

Ok, here's what going on ...

1.  The 1250 requires a minimum of 20.0w of power.  You can operate the 1250 in 15.4w of power if you are using the correct IOS.  When you don't use the correct IOS, one of the radios will go offline.  If you use the correct IOS, your 1250 will disable the two highest MCS rates.

2.  You have a 3560E.  By default IOS, your switch cannot go above 15.4w PoE.  If you upgrade your IOS to a minimum of 12.2(48)SE, then you should be able to enter this command into the interface "power inline auto maximum 20000".

3.  Your WLC is running 5.X????  Please avoid using this firmware as this is THE MOST buggy firmware available.  Upgrade to 7.0.240.0 and you'll also see the benefit of your 1250 AP able to operate at 15.4w PoE.

Check the Poe interface power status from the switch using

sw#sh power inline {fastethernet | gigabitethernet} slot/port. AP s are down as the input power to them is probably low.

follow the link for AP poe config

Here is my 2¢

You actually need to remove one of the modules or else the 1252 will run in a reduced power mode.  If its working, I wouldn't worry so much about that log message.
