AP1200 IOS 12.2(15)JA,
in a non-WPA/CCKM implementation,
What is the difference between WEP: Mandatory with MIC and PPK - and Cipher: TKIP + WEP128?
Does the TKIP + WEP128 Cipher package provide PPK and MIC? The documentation for this is a little sparse.

TKIP + WEP128 is what is known as WPA Migration mode. It provides WPA TKIP which assumes WEP is incorporated and also 128bit WEP encryption scheme for non WPA clients.

Have you gotten it to work?
Once configured all Cisco clients using LEAP and non-Cisco clients using WPA-PSK cease to be able to authenticate.
I've given up on it and am configuring all clients to use WPA-PSK only for now.
