WLC WebAuth Sample Page

I'm trying to create a customized WebAuth page for WLC 4402, latest image.

Is there a sample/template page available that I can use as a starting point. I found one in http://www.cisco.com/en/US/tech/tk722/tk809/technologies_configuration_example09186a008067489f.shtml

but it doesn't work.
