Environment: PBX in basement with fiber feeder to second floor. PBX does not support an IP conference room phone but all other desk phones are VOIP connected to a card in the PBX. I need to get an analog extension to the second floor.
How do I configure the SPA 3102, connected to a line on the PBX and ethernet to switch and the SPA 3102 on the second floor connected to switch and analog phone?

If your PBX is digital and supports sip you may be able to just use an analog terminal adapter (ATA) and setup a new extension on the PBX.
If this is not possible, it is possible to use a SPA3102 to interface an analog phone line from the PBX and extend that phone line to a distant analog terminal adapter such as another SPA3102 or other analog voip terminal adapter that supports unregistered calling. Using a SPA3102 is not as elegant or professional as the commercial market TC1910 which was referenced and is sold specifically for this analog phone extension purpose.
Configuring 2 SPA3102's "back to back" has been around for some time. Search on "SPA3102 back to back" and you will find help like the detail in this thread

Basically you configure the PSTN Line Tab of the SPA3102 to attach to the analog phone line and automatically communicate with the distant ata using direct unregistered ip dialing. For an outgoing call from the PBX, upon receiving ringing from the PBX the SPA3102 will automatically call the distant ATA. For an outgoing call from the distant ATA, when the caller takes the phone off hook the ATA automatically dials the SPA3102 which will return a dial tone to the caller.
The distant ata is configured on the Line Tab as a voip phone.
A key settings on both units is Register: No, Make Call Without Reg: Yes, Ans Call Without Reg: Yes
Other settings are noted in the thread referenced above.
Calling is done using the ip address:port of the opposite adapter. You can setup the adapters with static ip addresses.

Sorry, but I don't fully understand neither the description of your topology nor your intentions.
Let's allow me to rephrase.
You need to connect particular analog phone on second floor.
You have no analog wire from PBX to second floor. OK, but it is unclear to me what type of connection IS available on second floor. It seems you have Ethernet line here. Is it true ?
If so ...
You claimed the PBX is VoIP capable ("desk phones are VOIP connected to ... PBX"). So connect particular analog phone thru an Analog Telephone Adapter thru Ethernet you have to the card in PBX you mentioned.
I'm unsure why you are trying to craft own "POTS-Ethernet-POTS" extender, but may be I misunderstood your goals. If you need such kind of extender, you should consider to buy the one - like TC1910 or so. According your question I assume you have little or no experience with SIP. It will be hard to you to craft your own extender and you will not be able to solve issues related to it.

I finally got this working using the link offered by Howard. My mistake was using the Ethernet ports vs. the WAN ports. Also, had to change the line voltage from 30 to 23 otherwise got fast busy on the phone attached to the remote SPA.
