2921 EVM-HD-8FXS/DID first two ports go into FXSLS_PARK state

Hello,
We are currently in a rollout of ~200 locations in which we are deploying 2921's with EVM-HD-8FXS/DID, EVM-HDA-8FXS/DID, and PVDM3-64 we are running IOS 15.1(2)T3,. The 2921 interconnects our KSU, modems, faxes from a SIP trunk provided by our carrier. We currently have ~24 locations deployed over the last 2 months and are starting to see an issue (currently 3 locations) in which port 1/0/0 and 1/0/1 get stuck in a FXSLS_Park state, it's important to note, that these two ports take a majority of the calls. Occasionally a reboot of the router will resolve the issue (shut/no shut does nothing), but now even a reboot is not resolving the issue. We have also disconnected the amphenol cable that runs to the existing equipment and shut/no shut, reloaded the router and the port still continue to come up in a FXSLS_Park state. it's important to note, that these locations were early deployed locations and have been running fine for over a month and then progressively got worse. If this is a hardware issue, that concerns me, an 12% failure rate is not acceptable.
Any suggestions you can provide with be greatly appreciated!

Update:
Cisco TAC replaced the EVM-HD-8FXS/DID voice module in the router and issue is still occuring, now on other ports as well.. Next an attempt to replace the PVDM at this location.

Did you get a resolution to this issue?
We are seeing the EXACT same issue with these EVM cards on mulitple 2911s with PVDM3-64s and 3925s with PVDM3-128s.
Any help would be appreciated!

I ran accross the bug below. Altough we are seeing this on other ports besides first two on the HA and HDA 8/FXS cards. Also we are running different DSPWARE versions than described. Sounds like this may be wider in scope than what Cisco believes.
