Hi Paolo Bevilacqua & Anas Abu Eideh
I have figure the issue is not cause of IOS.
The is because i need to manually add in the DN for the conference purpose.
Below is the good article and explanation.

thank you.

Hi!
Sorry for a bit later answer, but it may be helpfull for people facing same issue.
The point is in that DN with "conf ad-hoc" should not be tied to phone, but just left alone and there should be at least one such DN it config for conferencing to work. When someone creates conference CME uses these DN to do the bridging - it's just internal resourse and shoudn't be tied to phone.
Config is following:
! // this is fake DN for conferencing
ephone-dn 501 octo-line //octo-line for bridge up to 8 calls
number 3599
name Conf1
conference ad-hoc
preference 1
!
! // this one is real DN on phone
ephone-dn 7 octo
number 3511
label Mohhamed Abu Ibn Abdul
name Mohhamed Abu Ibn Abdul
corlist incoming unrestricted
!
ephone 7
button 1:7 // <-- DN 7 here, at it has no "ad-hoc" option.
restart
!
Of course, all other config has to be done:
1. voice-card X
dspservices dspfarm
2. dspfarm profile for conferencing
3. sccp config to use dspfarm profile
