Intermittent incoming call drop out on answer.

Call flow as below:

ITSP--SIP--CME--SIP-9971

2900-universalk9-mz.SPA.152-4.M1.bin

Debug ccsip message and ccapi inout as attached for both working and failed calls.

Are you using a parallel voice hunt? If so, try without. Also, try with SCCP phones.

site only using 9971 sip phone, i cant mack around with sccp phones.

As above, I think you are using a parallel voice hunt group. Not good, especially with buggy SIP phones.

this issue is finally resolved by upgrading IOS version which accept by TAC as bug.
