CUCM 8.6 - Outbound SIP trunk to ITSP - Anveo Direct

I'm looking for assistance in creating an outbound SIP trunk to Anveo Direct.

I have a trunk created in CUCM to a 3845 gateway configured for CUBE.

2 issues that I have:

I need to add a prefix before sending to the ITSP    

     [PREFIX]PHONENUMBER@SBC.ANVEO.COM

I also need to adjust the IP address within the SIP packet to use out external IP address instead of the internal address.

Any suggestions?
