i have some trouble with 4 cisco ip phone 7821 who renew ip address after reboot, paste the log of the phone next:

i think that the the phone are damage, i am correct?

Hi Jorge Alberto,
if my understanding is correct, you have more than four 7821, but only these four are having problems, am I correct?
Did you try a Factory Reset (check the link Perform a Factory Reset)?

yes i tried factory reset but the same result also i changed the firmware the problem solved for about 1 hour. after the problem continue

ok, and you have others 7821 that didn't have the same issue?

yes, around 200 more phones only 4 phone have the issues

Ok.
what happened if:
1) you configure these phones with DHCP disable and after a couple of hours change it to DHCP enable?
2) you configure the following:
In CUCM Administration > Device > Device Settings > Common Device Configuration: Select the 'Common Device Configuration' IP Addressing Mode: IPv4 Only and associate it to these 7821
