Hello everyone,
I have another strange issue with an IP phone that doesn't want to ring.
There are 4 analog lines plugged into an FXO card in the router, and all are configured the same. The lines are PLARed to the phones - and all but one are working.
I've run a debug vpm signal and received the following output.

I notice the 'no ring detected' in the output, and I'm wondering what the problem could be.
The show voice call summary output shows the VPM STATE as FXOLS_RINGING, but the phone itself doesn't ring.
Is there anything someone sees that I'm missing?
Any help would be appreciated!

Is this Callmanager or CME ?

It's CME - very small office with a couple phones.

When you make internal calls between phones does the phone ring ?
Try a csim start
PLAR all 4 FXO lines to the ip phone in question and call each FXO number from PSTN, see if all 4 fail or one but all fails...

I've done what you suggested.
Internal calls don't have any problems, they can call to eachother through extensions.
I performed a csim start and it went through successfully, meaning that it gave me no errors. I tried a couple invalid numbers and it gave me errors but when I typed the extension it went through.
I PLARed another line to the extension, and called the number, and it went through! Well, ofcourse I had to PLAR it back to the original line...
So does that indicate anything? I'm thinking of the FXO port being faulty but the phone doesn't have any problems making calls outbound. It just doesn't ring when someone dials that number from outside...

Ok - I got the problem fixed.
The problem was with the FXO card itself. I think the DSP on the board was probably defected.
We ordered a new card, plugged it in, and bam... started working again.
