Multiple Cisco 3560's showing same error : %SWITCH_QOS_TB-5-TRUST_DEVICE_LOST: cisco-phone no longer detected on port

Here is the error that shows in the switch logs:

.Apr 19 10:01:43.402 MDT: %SWITCH_QOS_TB-5-TRUST_DEVICE_LOST: cisco-phone no longer detected on port Fa0/33, port set to untrusted.

.Apr 19 12:52:07.899 MDT: %SWITCH_QOS_TB-5-TRUST_DEVICE_DETECTED: cisco-phone detected on port Fa0/33, port trust enabled.

This error is across multiple switches. The phones(all 7912's) mysteriously reboot and then the computer connected to the phone will get a 169.254.x.x and cannot reconnect to the workstation VLAN through the phone until the computer is restarted.

The phone load is:CP7912080004SCCP080108

Here is an example of one of the ports that is having the issues configuration:

Anyone have any ideas?

Hi

Could well be a firmware issue on the phones, or a problem with the IOS on the switch. I guess you haven't tweaked the cdp timers on this switch?

Did you notice when this started happening (i.e. after a CCM upgrade, or switch software or config change)?

We recently upgraded Call Manager to v. 7.1.2 from v.5.

I am going to upgrade CM to 7.1.3, hopefully this resolves the issue. I tried upgrading the phones firmware to 8.0.4 and this did not resolve the issue

Did your upgrade resolve the issue? We're running CUCM-Business Edition 7.1.5.10000-12 and we are getting sporadic phones rebooting. I've upgraded the phone's firmware to SCCP45.9-1-1SR1S (the phones are 7965s).

The phones also reboot while users are on a call. I've reviewed the SDL traces along with Wireshark captures and can't find much information. I'm working with TAC currently, but they haven't been able to track it down yet either (1 week and going). My next plan is to upgrade to CUCM 8.0(3a).
