Phone Template lose it configuration

Hi all,

I've been configured Phone Template with SURL on the 2nd button.

When I'm adding new phone from template it's taking the SURL just like It's supposed to be,

but when I try to add another phone from template there Isn't SURL, It's gone from the template.

The CUCM is version 11.5.

Try to make a copy of the first (good) phone and see if that makes a difference.

If the SURL button is there on the copy check the content.  You may need to use Related links drop down and Subscribe/Unsubscribe Services to add the service.

Not sure if the actual service copies over, probably just the SURL template and button.
