Remove Add on module from button template

On my 8851 when selecting Modify Button Items on the Device page - I show this:

I'm trying to have the first SD, 68916 appear on the top right key (#6). Instead, I show a handset icon. If I press that, I get dial tone. Another 8851 template doesn't have ----- Add On Module(s) ------, 6 and the associated speed dial appears after 5. That phone's button #6 on the upper right is in fact a speed dial. No modules are shown in either on the Device page. How do I remove that from the first Modify Button Items display to make it look like this?

I even tried assigning the bottom template, save and apply config, restart the device and I still don't see a 68916 speed dial on button #6, its the phone icon. Pressing that gives dial tone. The icon below it looks somewhat like a clock, pressing that is the same result as pressing Settings then Recents.

If you're trying to put it on the top right on the phone, you have to change to Enhanced Line Mode in the Product Specific Configuration Layout of the phone.
