Sample configuration for CUBE on 2801 ?

I am looking a sample configuration to use for Cisco CUBE running on a 2801 router.  For cost reasons we will be going with the 12.4 code train since 15.0 requires substantially more dram and flash than we have in the router at this point.

We are using Surewest/Consolidated for our SIP Provider.  I have been trying for several weeks to get a sample configuration from them but they havent been able to come up with anything.  They are using SIP gear from a company by the name of INNOMEDIA.

If anyone has a CUBE connector to SureWest/Consolidated, I would appreciate it if you might be able to share a sanitized version of your config.  We are currently set up for a 6 call paths.
