do I need to create dial-peers for in/outbound calls?

also, can I do a translation pattern to turn the inbound DNIS 5000 to internal 101 like this

Yes you can have SRST on an MGCP gateway as follows: http://www.cisco.com/en/US/products/sw/voicesw/ps2169/products_configuration_guide_chapter09186a0080557e3a.html

I've never set up SRST on an MGCP gateway though as if running SRST I always make the gateway H323. That way it keeps configuration simpler and you can easily use the alias commands to route inbound calls.
