Unable to receive incoming calls out of the Auto Attendent

Unable to receive incoming calls out of the Auto Attendent.   Flow is Pots Line - FXO - AA - IP Phone

When an option is selected and or a extension is dialed, call just rings and which doesn't make it to any extension.  No voicemail landing either.

What is the auto attendant and how is it connected to CUCM? Assuming Unity Connection with a SIP trunk, ensure the Rerouting CSS is set; unsupervised transfers invoke a SIP REFER.
