OI have a cisco 2800 and need to achieve voip on port 5060, mypbx ip 10.1.0.191 and cisco ext ip 41.215.180.46. Please help

OK, in first place you need to configure CME on your ISR (refer to the CME admin/config guide for any task), then, I'm assuming the other end supports SIP trunks, so you'll need to configure your SIP DPs to it. IP Phones dial DNs, not IPs.

You may want to ask this question in the appropriate section "Routing and Switching" since your question is related to NAT and not Cisco Voice.
