Cisco 2960X voice VLAN

Cisco 2960X I am facing problem-related to voice VLAN. It's up down frequently but data VLAN is working fine.

Kindly find attach notepad for better help.

I am not able to open/download that text file;

how is PC/IP Phone connected ? is PC connected via Phone?  they share switch port ?  aka, voice VLAN and data vlan on same port.

check PoE on switchport and overall power status of switch (Wattage), check cable with switch TDR tool,
