ios load error "cannot accept files longer than 45 char"

upgraded c5915 router and the ios crashed getting a ELF 0x0 error.

I reloaded the ios .bin file via xmodem however after it finishes transferring the file I get the below error during saving the .bin file to flash

error -1 "cannot accept files longer than 45 characters"

it is the same ios image and filename I was successful loading onto another c5915 router. any suggestions?

where is the TFTP Server ? are you able to have reachability to TFTP.
