I have found that if I have a Call Handler with a certain extension and delete it, I am never able to add a subscriber that uses this same extension.
I am on Unity 3.1 Build 3.1(5). Any fix for this? I'd really like to use these extensions that were once linked to Call Handlers since they are DIDs.

so this happens consistently (i.e. you could reproduce it for me to look at)? I'd be interested in taking a look if so.
Can you try the latest dbWalker here:
http://www.ciscounitytools.com/App_DirectoryWalker3.htm
and see if it turns up anything about those extensions being stranded in the DTMFAccessID table? I've added a bunch of new checks for DTMFID conflicts and issues in recent versions of dbWalker, it'd be interesting to see if it points anything out here.

It turned out to be an Exchange issue, not an extension issue. Thanks for the prompt response!
