CCM 4.1(3) with Unity 4.0(5).
When the failover server is active I can dial into Unity by pressing Messages button or calling pilot number, I can also leave a message from within Unity. The problem is when I dial an extension that should forward me to voicemail, I get fast busy. I never see the call hitting Unity. My integration is set up as following:
CERO-primary with 34 ports
Hunt options:
No Answer: Skip remaining members, and go directly to next group
Busy: Try next member, try next group in Hunt List
Not Available: Skip remaining members, and go directly to next group
LIBE-primary with 34 ports
No Answer: Skip remaining members, and go directly to next group
Busy: Try next member, but do not go to next group
Not Available: Skip remaining members, and go directly to next group
CERO-Fail with 34 ports
No Answer: Skip remaining members, and go directly to next group
Busy: Try next member, try next group in Hunt List
Not Available: Skip remaining members, and go directly to next group
LIBE-Fail with 34 ports
No Answer: Try next member, but do not go to next group
Busy: Try next member, but do not go to next group
Not Available: Try next member, but do not go to next group
In the hunt list the order is as above, however I tried with only failover groups and still the same issue.
On Each Unity I have 2 integrations with 36 enabled ports, the last 2 ports are MWI.

Chris,
What does the CCM trace logs indicate ?

I've attached a snippet of the trace, the only suspicious thing I see are the following lines:
CCM|Forwarding - awaitingCallResponse_SsRejectionInterceptInd - CFB processing. party= 33556890, callKey= 0x125|<:CGCCLUSTER><:10.233.149.122><:2><:10.233.59.78><:SEP00152B36091D>

What does the "no CFB dest or CFAP dest - Clear the call" mean? Why would it be looking for CFB???

I got it resolved.
Kudos to TAC for helping me fix it, it was a CSS/Partition configuration issue.
