I'm using CallManager 4.0(2)a and recently the CFwdAll button on handsets stopped working. We use 3 digit extensions starting with 2, but if I try to call forward my phone to any extension I get a congestion tone after I enter the third digit.
Where can I begin investigating this problem?

Check SQL replication. Use DBLhelper.
If all is ok restart DBL service.
Otherwise open a TAC case to diagnose the problem

Also try clearing the call forwarding tables as per this link
