How does ER identify the location of a call when the calling number is a DN which appears on more than one phone? For instance, say a secretary at the North East corner of the first floor dials 911 from a DN on line 4 of her phone, but that DN is also assigned to line 1 on a Manager's phone located at the South West corner of the second floor. How does ER determine which location the PSAP gets notified of?

The PSAP gets the DID configured on the ERL of that particular phone unless you have ERL created for each and every phone.
As far as E911 Server is concerned, you fill in the location details on a port on a switch. So when some phone dials 911 the location details that was manually entered by you on the port where the IP phone is connected goes to the E911 Server and to the inhouse Security personal.
