There are a couple of ways to do this.
1) If the server you are running CCM on has a PCI card, you can install a SoundBlaster card and connect the cd player to the line in.
Here's a link with some information;
2) You can use an external USB sound import device, such as the Telex Communications P-800 USB Digital Audio Converter. This connects to a USB port on some of the newer MCS servers and is basically a USB sound card. Note that this device is not supported by TAC. Search the forums, and you'll see a few people using this device.

It may not be TAC supported but atleast in the OS 2.5 Release Notes is says it is supported (or atleast tested by someone at Cisco at some point):
"Support for the Telex P-800 USB sound device to support music from an external source for the Music on Hold feature in Cisco CallManager on the MCS-7825H-2.2-EVV1, MCS-7835H-2.4-EVV1, MCS-7835I-2.4-EVV1 MCS-7845H-2.4-EVV1 and MCS-7835-1266 platforms"
