I have recently run into a problem where a user picks up a phone to make a call and there is no dial tone for a few seconds. This also happens on inbound calls both internal and external. A user tries to answer a call and nobody is there. Then after a few seconds the person will be there. They are also having an issue with transfers being lost. related?
A reboot seems to correct the issue for a week or so. They have no disk space issues. They are running 3.3.4. Any suggestions?

One thing you should check is that the "voice rtp send-recv" command is configured on all H323 gateways. It's a global configuration command. This may help your issue where there is a delay before you hear a caller.

Most probably it might be a high CPU issue on the CCM. I know there is an internal DDTs open for delayed dialtone and, I think it will be fixed in CCM v 4.1
