Hello,
My setup is CM 4.0(1)sr2 with voice gw 3745 IOS 12.2(15)ZJ3. Using H323 between CM and voice gw. When PSTN caller calls to IP Phone, the IP Phone user can't hear caller's voice for the first 3 seconds, afterward the conversation goes two-way. However, caller can hear IP Phone user's voice since the call is connected. This is happening to all IP Phones user for incoming calls. Outgoing and internal (between IP Phones) calls are not affected.
Have someone experience this before? What could be wrong in this case? I welcome you to share your experience, advise, or thought, etc.

for incoming call, what PSTN line are u using (e.g. FXO, DID, BRI or PRI?)

do you have these 2 commands?
h323-gateway voip bind srcaddr
voice rtp send-recv

For connection to PSTN, voice gw use E1-PRI.
Yes, I have h323-gateway voip bind srcaddr configured on FE interface (only one interface active) and voice rtp send-recv on global config.
