There are a bunch of things this could be. Can you post your config please? Do you have 2 NICs enabled on the machine or 2 IP address assigned to one interface on the Unity? Does this only happen for external calls? Does this ever happen on calls to IP phones?

I am running into the same problem. Internal calls (ip phone to unity) work fine. However, when you call in from the pstn, you cannot hear unity, but can record. We hav only one ic set up on the unity box. We are using g711 as the protocol.
