I get a strange output from the 1750 with ios 12.1(5)T8 as follow:
VOIP:
ConnectionId[0xA5BCD5C2 0xBABA01DF 0x0 0x9EF43E8]
IncomingConnectionId[0xA5BCD5C2 0xBABA01DF 0x0 0x9EF43E8]
RemoteIPAddress=10.254.255.6
RemoteUDPPort=0 <------------------Why this port is 0
RoundTripDelay=85 ms
SelectedQoS=best-effort
tx_DtmfRelay=inband-voice
FastConnect=TRUE
Why the voice port is 0. Is it should be 13xxx to 32xxx? Is that the ios or router have the problem? Anyone know it?
Thanks!!

When I use the udp voice port to do the QoS. Is it would work properly? Because I find this problem occur at 12.1(5)T8, 12.1(5)T10 and 12.1(5)T12.

I don't quite get the question...
the rtp stream can be on any udp port from 16384 32767, it is an arbitrary decision. If you configure QOS based on this udp port range, it will classify the traffic correctly. I would reccomend running 12.2 mainline IOS for IOS platforms.
