Reader Decode Failed

I have noticed when reviewing Events in our CPAM installation that we are receiving a lot of Reader Decode Failed messages from one of our readers.  The second or third attempt to badge in successfully allows access.  Most of these attempts are miliseconds apart.  What is the appropriate way to troubleshoot this behavior? 

The reader in question is a HID MaxiProx 5375.  The reader is independently powered so we are only getting D0 and D1 from it to the gateway.  There is a Ditek DTK-LVLP series low voltage line protector between the reader and gateway.  Any help would be appreciated.

We've seen this happen as a result of voltage spikes from sources near the door like the lock solenoid for example. 

It sounds like you've tried to mitigate that sort of thing, but if you're seeing events happening miliseconds apart the first place to look would be the wiring. 

See if the errant signalling coincides with other events going on around the door that could be sending voltage down the line via induction.  The gateway could mistake that for an incoming signal from the reader.
