Exact IOS used? Need to stay at the latest to minimize chances of bug/crashes.

This can be the problem with the IOS or the harware.
Need to know more on the modules used on both sides.
Show verison.

I dont see any problem with the hardware.
IOS I cant comment right now but there is a error in router A in show log.

This means there are chances of bug in this ios.
Would like to know if you give show flash. Do you see crash info. Can you pull the crash.
Please disable cdp on one of the link on both the interface

Well the interface on router B is shut down which is causing the router to crash ?
Is it possible for you to disable CDP on all the serial interface and try to check.
Also if it does crash show tech-support and show verison would be useful.
apart from this i cant think anything but the IOS to be at fault

I am seeing that the ser0/0 on router B is config with loopback. Why ? have you tested the link with loopback command on it ?

oh that was for testing with carrier. I've removed that now and tested again, still same problem.

I dont see any problem with the config nor with hardware.
IOS upgrade would be one of the option to see if the problem gets resolved.
Before that i would suggest to swap the WIC cards and the config on one router each first.
1. Router A. Swap the cards. change the config.
check what happes.( If it does not work revert the config back and cards back)
2. Router B. Swap the cards. Change the config.
Check what happens..( If it does not work revert the config back and cards back)
3. swap the cards and config on both the routers and check.
I would have followed all this steps to get to the root casue. This will help us know if this is a slot problem on any of the router.
If the above troubleshooting comes out with the same results with one link down. Then IOS upgrade would be needed and if still then TAC CASE.
