Is AppX license required for IP SLA on Cisco ISR 4000?

In the datasheet for ISR 4000 IP SLA is mentioned only in the AppX bundle.

Is IP SLA also available with the Security license or do we have to buy AppX to get it?

you will need the AppX for all functionalities of IP SLA

It says there that with appx you additionaly get IP SLAs LSP Health Monitor - (APP/DATA license only).

Do you perhaps know what it is used for?

 If some DC site/NOC team location router with APPx license will be used to monitor all other office routers and links then also all sites need Appx license? or only site router with NOC would require Appx license (IP SLA initiator) and rest would be acting as IP SLA responder? 
The router used for monitoring and under monitoring will be 4k series.

For IPsla on c4000 series, You just Security feature license enabled, pls check show version License.

the command syntax is bit different due to XE ios, I just recently configured and it is working like charm.

track 1 ip sla 1 reachability
ip sla 1
icmp-echo xxx.xxx.xxx.xxx(destination) source-ip xxx.xxx.xxx.xxx(sourceIP)
frequency 15
ip sla schedule 1 life forever start-time now
