Normally from the supervisor, I can find the card as follows:

From the supervisor I enter sess 15 and I can acces this module.
Now, I get:
sess 15 [enter]
Module 15 is not installed.
Circumstances as follows cause the module to be inaccessible:
When I tried to test upgrading the catOS and IOS on the switch B,
I found that the bootflash was full and the previous IOS had
to be deleted to make room for the new version. I deleted
the IOS file from the bootflash, but the space was not made
available. I then ran the "squeeze" command. However, this
resulted in an error message about the flash is now in a
inconsistent state. I proceeded to reload the IOS and
eventually reset the supervisor card.
After this sequence of events, mod 15 no longer
appeared.
How do I recover this?

Have you tried session 16? Can you post the 'show mod' ouput?

Hello and thanks for the reponse.
I am able to get to the rommon prompt at
step 2 from which I can proceed to step 13 and issue the dir bootflash: command
cat6500B> (enable) switch console
Trying Router-15...
Connected to Router-15.
Type ^C^C^C to switch back...dir bootflash:
