Looks for me you locked out due to some AAA config (this is guess only)
Contact TAC (if you do not have ISO image to boot and recovery)

The easy way is the Disaster Recovery method. But you will need the IOS image. Save it on a usb, reboot the NCS 540 and press the break sequence, select the usb and boot the ios from the usb.

How do perform a password recovery on a N540-24Z8Q2C or
N540-ACC-SYS router? Also, need the documentation on how to reset the router to factory defaults.

If you want to perform a password recovery you have to do the usb boot method. To do this you need the image of the firmware save it on a usb. Connect the usb on the router and power cycle. Then use the break sequence. (For me control+ c always work) and from the menu select the usb and boot it. This will install the ios image tha you have in the usb. And the router will boot up factory default. So you will lost any configuration the router had.
If you can access the cli with a user with admin permission you can use commit replace command from the config terminal. This will erase all the configuration and boot up as factory default. Do this from console and not from remote access cause you will lose connectivity.
You can find the documentation for the password recovery under NCS 540 Disaster Recovery

Do you have the specific document, because when we try the break sequence with ctrl +c it does not break into a menu. The model of routers are N540-24Z8Q2C and N540-ACC-SYS running IOS XR 6.3.3. If you could proved the exact steps or specific documentation for doing a password recovery/factory reset that would be very much appreciated. Thank you.

For me ctrl + c works but documentation said is ESC button. So try that.
