We have here two Cisco Catalyst 6509 running Cisco IOS in hybrid mode. The details are:

We tried to install a new module, exactly a WS-X6516A-GBIC. The chassis didn't recognize it.
It is very strange because the documentation says that this module (WS-X6516A-GBIC) is supported by the chassis.
I think that the problem is a software problem, maybe we need an upgrade but I am not sure. Another option could be to migrate the configuration to native IOS mode.
Can anyone give a clue to solve this issue?

Your issue is definitely software related. You must upgrade the sipervisor software to solve it. I found a URL that confirms that ver 7.6 does support your module:

It might be possible to find older versions as well but ideally you would want to upgrade to a version that is stable and not too old.
Migrating to IOS is an option as well but the IOS typically requires more supervisor memory than CatOS.

Also, to add to the previous comment, note that 7.6(4) and later releases need 128MB DRAM.
