I inherited this device, did a factory reset and logged in to see what revision it is:
- Firmware Version: 1.3.0.62
- Boot Version: 1.1.0.6
Pretty outdated!
I read in the release notes and other forum topics that I should update the boot loader and firmware to 1.3.5. The only download for this is 1.3.5.58, and it is a folder full of MIB and MY files.
The release notes are written as if they expect me to have an image file or two. What do I do with these files? I'm having trouble finding the binary image files.
Can anybody lead me to the binary files, or instructions on how to use these MIB files?

After a few dead-ends I found some answers here:

I heard of this free TFTP server in another post: http://tftpd32.jounin.net
Download and install that.
For simplicity I created a folder c:\t1 and put the firmware file into it.
I eventually figured out there are TWO sets of firmware file download pages, and all but the latest firmware download links I had used took me to the MIB ones. One just has to (at the top of the page) click on the parent page (https://software.cisco.com/download/home/283771818/type) and select the Smart Switch Firmware download (not Smart Switch MIB).
From the firmware page, click EXPAND ALL, the select the 1.3.5.58 version: (https://software.cisco.com/download/home/283771818/type/282463182/release/1.3.5.58)
From the list, download:
Sx200 Firmware Version 1.3.5.58, (Sx200_FW_Boot_1.3.5.58.zip)

Unzip and you should get TWO files. Move the boot loader (sx200_boot-13506.rfb) to folder c:\t1 (or wherever you set up the TFTP server to use). Its handy to COPY the full file name into your clipboard for the next step.
In the switch, go to the UPGRADE screen. There is an important step coming that will drive you crazy if you miss it!
After reboot my Boot Version has changed to 1.3.5.06

I updated the firmware to 1.3.5.58. Based on what I read, I was expecting the boot loader update to be slow, but it was only 30 seconds or so. It was this firmware update that ran for nearly 5 minutes!
Now I will try the latest firmware: sx200_fw-1494.ros
And after a reasonable 1-2 minutes I'm back up, and my firmware now reports 1.4.9.4
Now I just have to wrap my brain around how to configure this beast!
Thanks to all those who have posted questions and help in the past, I was able to (eventually) use your posts to self-answer my problem.
Since it seems some old links are winding up in the MIB downloads, I expect more people will get trapped like I was. I leave this post in the hope they find it quickly.
