S14-DPAS-SW10#

I had two of these in a row now.  It happened when I unplugged a device from the switch.  I am not sure if there is some sort of known issue with this switch or am I seeing a hardware failure.

S14-DPAS-SW10#show version
SW version 1.4.0.88 ( date 06-Aug-2014 time 16:55:55 )
Boot version 1.3.5.06 ( date 21-Jul-2013 time 15:12:10 )
HW version V02

Any advice?  Any more info needed?
