Hi, I'm experiencing strange issue with two switches 2960X-48TD-L connected in a stack.
Switches were running 15.0.2-EX5(ED) for quite some time. We upgraded to latest recommended version 15.0.2a-EX5(ED) recently.
Every time we try to login throught ssh or telnet since the upgrade has been done switches reload automatically.
Management can be still done through serial connection.
Here is the log message we can find in the console window when error occurs:
May 29 17:20:06.671: %SSH-5-SSH2_SESSION: SSH2 Session request from 192.168.1.10 (tty = 0) using crypto cipher 'aes256-cbc', hmac 'hmac-sha1' Succeeded

I can't find any simmilar bug for this version in the Cisco bug toolkit.
Anyone experienced simmilar error?

Hi,
The IOS you are running 15.0.2a-EX5(ED is the latest version available for the 2960X switches.
Can you try redoing the SSH again and test? You should still open a ticket with TAC.

The same problem happenes everytime we try to login to switch.
We downgraded software from 15.0.2a-EX5(ED) to 15.0.2a-EX5(ED) and issue is gone.
There must be a bug in 15.0.2a-EX5(ED) version.

Exactly same issue here.
I don't know if I should downgrade or upgrade the Switches.
It is happen to Singleswitches and Stacks.
I have open up one TAC case and hope to receive Answer quickly because this issue block my schedule.

Same problem with my single 2960X-48FPS-L after upgrading to 15.0(2a)EX5.
Shut off the VTY lines to see if the that is the problem and the switch is running just fine. Any ideas?

I had this same problem and after opening a ticket with TAC they said I was hitting CSCud90069 Failed ssh login with "login on-failure log" enabled causes A switch or router to crash or experience a spurious access when a user attempts to log into the device via SSH and is either successful, or fails to log in. I have updated to 15.2(2)E3 and no longer have the problem.

The same for me. Updates to a newer version (15.2(2)E3) and the problem was gone.
