2960x issue with USB while trying to install software bin file

Have a bunch of 2960x's and some of them are missing any software at all and some just need an upgrade. I have read about using fat16 on a usb stick with no more than 2GB's when in use with the rommon. I am still getting errors trying to get the system to do ANYTHING with the USB. Have tried different manufacturers for the USB. I have been at this for days researching this online. Is there some kind of trick to it? I would prefer to use this USB method instead of tftp or xmodem if at all possible. The error messages involve !CSW signature and saying that it needs to be formatted first. When I format it on the switch it just outputs more errors. Sorry if i'm not giving enough info, I can definitely give more if requested.

Its 2GB good to go as below :

 

Windows format file system: FAT

Allocation unit size: Default allocation size

 

Plugin the USB - Power off Switch ( connect console cable)

power on the switch

 

when you get  switch: dir usbflash0 :   ( what you see ?)

 

switch:format usbflash0 ( then copy files also good idea)

 

Note : post complete console logs to what is wrong here ?
