hello we have 2 x 9500 in a Stackwise topology 

recently we had a failure where the  2x SVL interfaces and the Dual Active Detection link failed 

therefore the switches went Dual Active 

in the logs there were 

"Error reading DOM data from transceiver"

this error occurred against all the interfaces around 3 hours before the Dual Active condition 

we run 16.12.3a -- I thought this type of  DOM error were fixed in this version 

Q has anyone seen similar behaviour

I looked at this bug ID earlier


I thought it had been fixed in version 16.12.3a on our switches


I agree on your suggestion to upgrade

In fact, 16.12.3a is listed on the fixed release. But, as the logs describes very closely the Bug behavior and considering that sometimes Bug persists after cisco anoucing the solution of it, I suggest upgrade as start.  Version 16 is already a bit older so doesn´t hurt upgrade to 17.
